How to apply
To ensure patients’ rights and privacy, please obtain all the related paperwork ready for application. Lack or photocopied identification will not be accepted.
1. Please ask the doctor to file the application during diagnosis and examination, and pay the fee for the application at the counter.
2. Proof of identity for application: ID card or passport (please use the household certificate for children).
※The trustees need to bring his/her ID card, trustor’s ID card and authorization letter, and filing out an application at the outpatient clinic.
Note
(1) Based on the principle of applying in person, please bring ID card or passport for application. When a minor applying, need to have legal representative’s ID Card or Household certificate.
(2) In the event trustees are applying, need to bring with him/her trustors’ or other legal representative’s authorization letter, and ID card for the purposes of examination and making a photocopy.
(3) Service Hours: Monday to Friday (8:00-17:00), Saturday (8:00-11:30). Applications will not be accepted on holidays.
(4) For patients who wish to pick up the document on the same day, the processing time is approximately 90 minutes.
